Second COVID Vaccine Booster Clinic Dec. 10

Because of high demand, we are adding a second COVID vaccine booster clinic on campus next Friday, Dec. 10. 

WHAT: COVID Vaccine Booster Clinic, Part 2

WHERE: Hogan Campus Center, Suites B and C

WHEN: 9 a.m. - 4 p.m. Friday, Dec. 10

WHO: All booster shot-eligible faculty, staff and students

You should be prepared to enter all your information at the time of registration, including photos of your health insurance cards. 

You will be asked to log back into the system on the day of the clinic to answer a few questions. After creating an appointment, you will receive an email from Pelmeds with a link to log back in. On the day of the clinic you must log in and answer the questions under the QUESTIONS tab, and print your consent form to bring to the clinic. 

Answering the questions and printing the form ahead of time will help speed up the process on the day of the clinic. However, we will have blank forms at the clinic if you are unable to print your form.

You should bring your vaccine card so you can have it updated with the date of your booster shot. If you do not have your original vaccine card, please bring a photo of your card.
