Forms for Clubs and Organizations

Below are some of the forms which now may be submitted electronically to the Office of Student Programs & Involvement.  Please read the instructions for proper submission guidelines

Meeting Room Lobby Table Form (.doc)
Hogan Table Reservation Form (.doc)

  • To reserve a meeting room on campus or a Hogan lobby table, simply download the Meeting Room & Lobby Table Form, fill it out, and email it back to spi@holycross.edu.  If you are using this form to book a meeting room, please only use this form for regular meetings.  If you are planning a special meeting or event which will be advertised to the campus community, you must use the Event Registration Form.

Event Registration Form (.doc)

  • To reserve space or officially register an event, download and complete the Event Registration Form.  When submitting the Event Registration Form, you must sign up for a meeting with one of the professional/graduate staff in the SPI Office.  To schedule a meeting with a staff member, please visit the RSO Service Center (Hogan 230) and sign up on one of the staff schedules posted in the center.  If you are unable to come to Hogan to schedule a meeting, please call the RSO Service Center at x3488 and an office staff member will assist you.  Once you have scheduled your meeting, email your Event Registration Form to the staff member with whom you will be meeting.

Listed below are some other forms which may be downloaded and printed for your convenience.  However, these may not be submitted electronically due to the necessary Co-chair and Treasurer signatures.  Please submit these forms to the RSO Service Center located in Hogan 230
