Health Services keeps immunization records for 10 years from your date of graduation. If you need a copy of your medical record or immunizations please fax (508-793-3610) a letter indicating the following:
- Name
- DOB
- Year of graduation
- Fax # or address of where you would like to have records sent
- Phone number where you can be reached
- Signature
- Date of request
